About Us - How We Do It?
While treatment providers make their clients or patients come to them, we serve our clients in THEIR environments (home, work, etc). We recognize that most clients' disabilities make a weekly therapy or doctor appointment just that--an hour that something happened in the provider's environment but doesn't transfer into the client/patient's daily world or living skills...without constructive skills teaching and support. We are clinically competent rehabilitation providers, and are a field-based staff, managing life's medical crises, distress events, and daily circumstances where and when they occur. As a result, our staff may be mediating an employer meeting, managing a hospital discharge plan, or conducting a family estate planning meeting all within the same day. We use a team-based approach to care management, bringing in different disciplines and skills as they are needed for each client. Serving clients through disability events, medical recovery and/or constructive life changes require us to constantly develop new and different resources to serve a client's changing needs. Historically, we have served clients for as briefly as a month ( usually providing strategic planning or crisis intervention) or as long as 10 years ( providing ongoing care management, illness management, and/or financial services). |
