As a result, we have a unique understanding of how a life with mental illness does not fit neatly into our present-day culture of home care, assisted living, and nursing care. We ensure that a lifetime of managing a mental illness is not made worse by obstacles to quality placement or care in a new setting. We provide both care-coordination and support services for those who are no longer capable to manage themselves in their home environments, including the best of geriatric psychiatry and community-based geriatric nursing and care managers. We also have our team of qualified elder law attorneys to ensure dignified living and access to public resources in guardianship and Medicaid-related advocacy. As resource experts in our community, we can identify . Aging with a mental illness can create more isolation, physical disability, and potentialfor self neglect; living alone with a mental illness only accelerates the risk for these events.
