Psychiatric Disability Care
O’Connor Associates, Inc has extensive knowledge and experience in the management of psychiatric disabilities. Our primary goals for clients are sustained outcomes from medical treatment and maximum functional recovery through rehabilitation and care management.
Our Outcomes Include: ♦ Resolving dependency-based barriers to acheive and sustain independent living; We are trained psychiatric rehabilitation providers, and teach our clients the adaptive skills necessary to minimize the functional disabilities created by a psychiatric illness. Our ethic in pursuing individual recovery goals is based in self-determination and least restrictive alternatives; as a result, we adapt to a client's changing goals, abilities, and needs. Our care management services coordinate historically siloed and seperate treatment providers. One of the key values of care management services is integrating the information and treatment plans of unconnected providers in a client's treatment (i.e. moving from inpatient care to day program to outpatient care, or the reverse). Our secure, server-based records management ensures a client's treatment team in an emergency medical event have all relevant records in real time. Our individualized recovery plans compliment ongoing therapies and psychopharmacological strategies by teaching clients how to maximize therapy and treatment benefits, thereby achieving better quality of life outcomes in various domains of living (employment, home life, relationships, etc.) |
